Lutheran Health Physicians Collaborates with Meridian Health Services

Fort Wayne, IN — Lutheran Health Physicians and Meridian Health Services are pleased to announce that the former Lutheran Health Physicians’ office, located at 2622 Lake Avenue in Fort Wayne, is now operated by Meridian Health Services with primary care services being offered in collaboration with Lutheran Health Physicians. The high-quality primary care that patients have come to count on will be complemented by a new and wider spectrum of integrated care specialties that Meridian has to offer.

Meridian Health Services is a regional, integrated healthcare system that believes in treating the “whole person” by caring for the physical, mental, and social well-being of people to help achieve their optimum health.

Primary care and pediatrics will continue to be offered by Lutheran Health Physician providers, Dr. Wen-Chou Tang and Dr. Houda Trabulsi-Younis. They will continue to see patients at the Lake Avenue location now operated by Meridian Health Services’ specialty practices of MeridianMD and Meridian Health Pediatrics.

Both organizations are committed to patients receiving the most efficient, effective, high quality healthcare possible. With Meridian Health Services managing primary care and pediatrics, patients will now have direct access to a broad range of whole-person health services such as behavioral health counseling, psychiatry, care management, community-based services, and dental services on site in the future. Through the use of telehealth, Meridian Health Services will also increase patient accessibility to an even wider range of physical, mental and social services, that would have not have otherwise been easily available to patients to meet their needs as well as the community.

Additionally, patients who are uninsured or underinsured will have access to a sliding fee scale and charity care to assure patient’s access to care is not limited.
