Kimberly Fraze, NP

I am a psychiatric provider in the Meridian behavioral health clinic in Fort Wayne. Prior to joining Meridian as a Psychiatric Mental Health Nurse Practitioner, I worked as a licensed pharmacist in both retail and clinical settings.

I graduated from pharmacy school in 2008, completed an accelerated Bachelor of Science in Nursing program in 2020, and earned my Master of Science in Nursing in 2024. I chose Meridian because of its commitment to caring for all individuals and its focus on the total well‑being of every patient.

I entered this field to serve an underserved population and to provide compassionate, comprehensive mental health care. My goal is for patients to feel heard, valued, and confident that they are receiving the best possible care.
